The inventory accuracy formula
Inventory accuracy (%) = (1 − |counted quantity − system quantity| ÷ system quantity) × 100, per SKU. Roll individual SKU results up, often weighted by inventory value, to get a catalog-wide figure tracked over time.
Most inventory inaccuracy in practice comes from ordinary gaps — an unscanned return, a substituted pick that was not logged, a receiving quantity typed from memory — rather than theft. Scanning at every point stock changes hands closes most of the gap.
