What is in the template

This supplier scorecard records, for each supplier and period, how many orders were placed, how many arrived on time and in full, the share of items rejected on quality, the average lead time and the price variance against the agreed price.

Use it in a regular review, such as monthly or quarterly. The aim is not to punish suppliers but to spot trends early, back conversations with evidence and decide where to consolidate or dual-source.

The sample rows are fictional and can be deleted. The file is plain comma-separated text, so it opens in Excel, Google Sheets and Numbers; add formulas for any calculated columns after you import it.

Columns explained

SupplierSupplier name.
PeriodReview period, for example 2026-Q3.
Orders_PlacedNumber of orders placed in the period.
Orders_On_TimeOrders delivered by the promised date.
On_Time_RateOn time divided by placed, as a percentage.
Orders_In_FullOrders delivered complete.
In_Full_RateIn full divided by placed, as a percentage.
Quality_Rejection_RateShare of received items rejected, as a percentage.
Avg_Lead_Time_DaysAverage days from order to receipt.
Price_VarianceInvoiced price against agreed price, as a percentage.
Overall_ScoreYour weighted score, out of 100.

Where a spreadsheet template breaks down

The rates and the overall score are typed values in the CSV. Turn them into formulas in a spreadsheet, and decide your weighting up front (for example on time 30, in full 30, quality 30, price 10), or scores cannot be compared between reviews.

The template holds figures but not the evidence behind them: purchase orders, receipts and rejection records still live elsewhere, and counting them by hand each period is slow and easy to get wrong.

What should a supplier scorecard measure?
At minimum on-time delivery, in-full delivery, quality (rejections) and price against the agreed price. Lead time and responsiveness are useful additions.
How often should I review suppliers?
Monthly for critical suppliers and quarterly for the rest. Keep the period and definitions identical so results can be compared.
How do I calculate the on-time rate?
Divide the number of orders delivered by the promised date by the number of orders placed in the period, and multiply by 100.
